Registration Information and Forms

 Registering for DIHA is easy. Follow these steps:
  1. Complete the registration form (PDF [PDF File])
  2. DIHA has changed its insuring body to Amateur Athletic Union (AAU) this year. Each player is required to have an AAU membership (annual fee of $14.00). Follow this link to the AAU membership page. Fill in the form using the following information:

    Step 1 -- Program = "youth program"
    Step 2 -- Membership Term/Category = "added benefit membership"
    Step 3 -- Sport = "hockey"
    Club Name = "Davis Inline Hockey"
    Club Code = "PAHO35XKA0" (4th character uppercase letter O, last character zero).

    Note: only one youth AAU membership per player is required--if the player
    already has a current AAU membership for another sport (baseball,
    swimming, basketball), they are covered for hockey. Also, if the player
    has current AAU membership for DHS hockey or NorCal Extreme, they are
    covered as well.
  3. Write a check (payable to DIHA) and mail it and the registration form to Norcal Indoor Sports, 1460 Tanforan Ave., Woodland, CA 95776
